- Priority 1 – a child/ren at risk of serious harm
- Priority 2 – a child/ren of two parents or a single parent family working/training/studying
- Priority 3 – any other child (includes any parent at home ie “home duties”)
Thank you for completing this form. Please note that submission of this form does not constitute a guaranteed placement of your child at the centre. Priority of access and placement is determined by compliance with regulatory requirements. All waiting list applications are processed in order of the date of receipt of the original application.

The information and personal details provided on this Waiting List Form are managed in accordance with the Privacy and Personal Information Protection Act 1998
